Samples of plants or crops can be submitted to the Nevada Department of Agriculture (NDA) Plant Pathology Laboratory for analysis and diagnosis. Please follow the instructions to create a Plant Sample Submission Form and submit samples.

Sampling and Submission Instructions

  1. Please create one submission form per plant species. A sample from a different plant species must be attached to a separate form. After submitting the form, print it to include with your sample submission.
  2. Samples should be collected on the day you plan to submit them. Individual samples should be taken from symptomatic tissue, or parts of plants, of the same species. Multiple samples can be taken from the same species and should be taped together with one form. Samples can be shipped or delivered to the following address:

  3. Email additional information such as disease or damage photos, cultural maintenance history, spray history, and other information not reflected in this form to with the subject line reflecting the 18-digit submission ID populated upon form submission.

CONFIRMATION OF ACCEPTABLE SPECIMEN



The NDA Plant Pathology Lab provides diagnosis of diseases in plants or agricultural crops, serving the general public, farmers, and government agencies. The lab does not accept nor have the capability to handle non-plant-related materials or substances, including but not limited to: materials from human bodies or clothes, animals, pets, building structures, open environments, animal nests, droppings, remains, household waste, or any unknown substances or powders with or without strong odor, for any purpose of identification other than disease diagnosis for a living plant or a crop.
* I confirm that the sample submitted is a plant sample for disease diagnosis and/or a soil sample for nematode testing, and that it does not contain any unacceptable material specified above.
